Clinical Aesthetic Services

 

Title:               Thermo-lo Milia

Policy:             All clinical aesthetic staff rendering client care will adhere to this protocol

Purpose:          To safely and properly administer Thermo-lo

Scope:             All licensed aestheticians rendering client care

Definition:       Clinical Protocol

 

Indications:  To improve appearance of milia.

 

Contraindications: 

1. Do not treat if client is Fitzpatrick IV-VI.
2. Do not treat if client has a sunburn. Cannot treat clients with frequent unprotected sun exposure or tanning bed usage.  Cannot treat tan skin.
3. Do not treat if client has a Herpes Simplex Virus (HSV) outbreak.  If client has a susceptibility to HSV, avoid common outbreak area with solution.  Can take prescription Valtrex or Zovirax prior to treatment, or immediately after treatment if beginning signs occur.  If client needs a prescription, notify Overseeing Medical Director.
4. Do not treat if client has significant product irritation.
5. DO NOT use on open lesions, rashes, burns, or suspicious lesions.
6. If client is pregnant, written authorization is required from her medical provider approving laser treatment during pregnancy. 
7. Cannot treat skin if client has been on Accutane (Isotretinoin) within the past 6 months.

 

 

Procedure:        

1. All clients treated will have a skin history consultation, sign a Thermo-lo Consent form during which time questions are answered, as well as discussing potential risks, complications, and anticipated results.  These completed forms will be present in the chart.
2. Any drug allergies will be noted on the front cover of the chart.
3. Review proper post-treatment instructions as well as product use after treatment. Provide client with typed home care instructions.
4. Photographs may be taken.
5. Client will lay in treatment chair, supine and in a reclined position.
6. Clinician to perform service in latex or non-latex gloves.
7. The client is properly draped, using linens that are laundered after each use.
8. The skin will be cleansed, with appropriate cleanser and soft gauze, removing all make-up excluding eye make-up.
9. Set machine to “manual” mode.
10. Insert a new tip into the handpiece.
11. Set the Intensity knob/setting between 6 and 7. Typically you will start low and increase as needed.
12. With the probe in hand, locate the milia to be treated and insert the tip of the needle underneath the white ball of hardened oil at about a 45-degree angle and pulse the current with your foot pedal.
13. Working your way around the lesion do about 2-4 pulses.
14. Using an extractor, gently scoop the white pearl/ball out of the skin. NOTE: there will be what looks like a hole in the skin. This is normal. Inform your client that this is the empty space the oil was occupying and it will close up in a few days.
15. If the white pearl does not budge, pulse 1-2 more times.
16. Continue treating desired areas as necessary starting on the right cheek and work counterclockwise around the face.
17. Skin will appear blanched in treated areas.
18. Apply moisturizer.
19. Apply Broad Spectrum SPF.
20. Give client post-care instructions.
21. Clean goggles with 70% alcohol.
22. Empty and clean steel water bowl with cavicide solution spray for 10 minutes.

 

Complications:

1. In case of complications or emergency, clients are advised to contact the clinic manager immediately. If clinicians cannot resolve the matter, our Medical Director, Dr. Shah, is to be contacted and available within 30 minutes or the client should call 911.
