COORDINATE RECEIVING CLINIC CLIENTS HQ & PH SUBMISSIONS FOR NEW ACNE CLIENTS 

Task

When a Holistic Acne Consultation is booked, it is mandatory the new client fill out a Health Questionnaire and Patient History form prior to their consultation.

All new clients are emailed a link to fill out the forms online via Zenoti.  These are to be filled out AT LEAST 48 hours PRIOR to their consultation appointment.  

To view a Health Questionnaire submission, navigate to client's Profile in Zenoti, then scroll over to "Consent & Treatment Records" 

Click the Arrow on Holistic Acne Consultation and check if the form has been filled by the client.  

To resend the link for the client,  right click on the appointment and click "Send Consent & Treatment Form."  

Once received, enters Patient’s Information in Guest Profile
